Spouses & DOB and/or DOD
All children at home & DOB
Address
Marital Status
Wedding Anniversary
Home number (if any)
Cell phone (for each adult)
Email (for each adult)

This information will be included in the church directory. If you do NOT want email/cell info included, please let me know and it will not be.

I use email/text as main communication for church, so I do need this information. It will be kept confidential.
